Worksheet, form, checklist, handout, scale, script, and protocol are not synonyms
A worksheet or form structures information entered by the therapist; a checklist supports a repeatable operational check; a handout supports discussion with a client; and a rating field records a self-report in context. A script or protocol contains procedural language and clinical decision rules. Calling every document a worksheet hides differences in purpose, authority, copyright, competence, and risk.
Searches for PDF, printable, free, template, or cheat sheet describe a preferred format, not clinical validity. A polished download can still be incomplete, unlicensed, outdated, taken from a training manual, or unsafe outside the assessment and treatment context for which it was written.
This library maps original resources to the therapist's actual task

Each resource has its own canonical guide and explains what the fields can and cannot establish. The library links to those owners instead of copying thirteen forms into one long page, so updates, translations, sources, and clinical boundaries stay consistent.
The library does not publish protocol scripts or protected assessments
EMDRIA lists professional books and resources that contain scripted protocols and summary sheets. This page does not reproduce their wording, training sequences, decision trees, proprietary worksheets, assessment items, scoring systems, or intervention scripts. Search visibility is not permission to copy clinical or copyrighted material.
A documentation aid cannot select a target, diagnose dissociation, establish readiness, determine pacing, interpret SUD or VOC, resolve blocked processing, demonstrate fidelity, or certify competence. Those decisions remain with appropriately trained professionals using authorized sources, consultation, supervision, and applicable standards.
Use the smallest resource that answers the current documentation question
Start with the task, not the download count. Leave a field blank when information was not elicited or a phase was not reached; do not turn blank into zero, unknown into normal, or a completed box into evidence of treatment completion. Avoid copying the same sensitive material into several browser drafts and records.
The printable forms have no submit destination, server storage, or autosave. Some browsers may restore form state after a reload, so use the explicit clear-fields control before leaving and transfer only the minimum necessary information into the authorized clinical record according to local law, policy, consent, and retention requirements.
Remote-session resources support workflow, not autonomous therapy
The remote checklist and contingency plan address the actual patient device, privacy, location, contact fallback, camera, microphone, stereo audio, BLS display, stop control, interruption, reconnection, closure, and follow-up. They do not prove that a remote session is clinically appropriate or that an emergency plan is complete for a particular jurisdiction.
EMDRSuite provides live video, therapist-controlled visual and auditory BLS, patient links, presence, pause and safety controls, reconnection, and therapist-authored notes. It does not deliver scripts, choose procedures, generate clinical findings, monitor risk, or replace the professional record.

Start with the question and authority of the resource, not the word form, free, PDF, or tool. A validated measure and an original documentation worksheet are not interchangeable.
Diagnostic interviews and symptom screens
Question it can support
Whether a trained clinician should assess trauma exposure, PTSD criteria, symptom severity, differential diagnosis, or another presentation with an authorized instrument.
What it cannot establish
No generic EMDR form diagnoses PTSD, selects treatment, replaces interview, or makes one measure valid for every population and language.

Are the EMDR worksheets free to use?
The public browser resources can be opened and printed without an account. Their limits and intended professional context remain part of the resource.
Can I save an EMDR worksheet as a PDF?
Use the browser's print dialog and choose Save as PDF when that is permitted by your device, workplace policy, consent, and record-handling requirements.
Does the library include an EMDR protocol script?

Do printable resources replace EMDR training or consultation?
